Authorisation for ALStrays to Transport Your Pet

Declaration of Owner not accompanying Pet during its journey

Please ensure you submit ALL the required details as without them we can't use the form, and once you have submitted it you can't edit it so will have to redo the form again once you have the necessary details.

With reference to my booking for the transport of my pet:*

Please enter your pets name above. If you are submitting for more than one pet please enter all the pets names above separated by a comma e.g. ABC, DEF, GHI, etc
Transport Date*

Please use the date we have published for the departure of the transport. You will find this information on the Schedule page
Pet Passport Number*

If you are submitting for more than one pet please enter all the pets passports above separated by a comma e.g. 123, 456, 789, etc

Please ensure you enter the country code e.g. ES, GB and don't leave any spaces between the Letters and Numbers i.e. XX123456789
Pet Microchip Number*

If you are submitting for more than one pet please enter all the pets microchips above separated by a comma e.g. 123, 456, 789, etc

Be careful, the microchip numbers have a lot of 0's in them!
I, the undersigned owner, declare that the following pet animals are not subject to a movement that aims at their sale or transfer of ownership and will accompany the natural person (Chris Marshall of ALStrays Transport ID MARSH607313CA9PL) who has authorisation in writing (in the form of owners signature on this form) from the owner to carry out the non-commercial movement on behalf of the owner within not more than five days of his movement:*

For pets being transported under the PETS Scheme we require the owners signature on the form as well so please let us know if you wont be handing over the pet to us so we can email you a copy to sign, scan and email back.

IF your pet is being transported under the TRACES scheme please ensure that the NEW owner (or foster person) completes this section.

Person Animal Being Delivered To*

Please name the person that the animal is going to be handed over to, and ensure that you provide their ID details below.
Third Person ID

If your pet is being handed over to a third person on your behalf (named above) please provide their ID.

Preferably a photo ID so we know handing over to the correct person. If no photo ID available then whatever you have I guess 🙂
Third Person Postcode

If your pet is being handed over to a third person on your behalf (named above) please provide their Postcode.
Owners Postcode*

The postcode of the owner where the pet is registered.

The owner is the new owner (or foster person) if transported under TRACES.
Owner ID*

The passport number, or alternative ID, of the owner of the pet.

The owner is the new owner if transported under TRACES.

Owners Digital Signature (with respect to the PETS Scheme I am aware of and comply with the 5 day travel condition) and confirm that during the non-commercial movement, the above animals will remain under the responsibility of the natural person (Chris Marshall of ALStrays Transport ID MARSH607313CA9PL) who has authorisation in writing from the owner to carry out the non-commercial movement on behalf of the owner*
